Our office is "paperless" with state-of-the-art computer software that utilizes an electronic medical record. When you call to make an appointment, you will be given a user name and password to access your medical history forms on-line. You can access this site at Digichart. This will be necessary to complete before your first visit to us. Once at the site, you double click on "Annual GYN" and complete the thorough medical history. This should take about 15 minutes to complete and will reduce your wait time at your visit. Please bring your insurance information with you.

If you are unable to keep your appointment, please call at least 24 hours in advance to reschedule.
